About the Role

Provides advocacy and assistance to patients facing biopsychosocial stressors and unmet health needs. The Case Manager monitors and collaborates with patients, care providers, and multidisciplinary teams to help patients achieve their goals.

Responsibilities

  • Assess patient needs upon referral, assist with priority setting for independence, and explain Seneca Nation Health System (SNHS) policies and procedures.
  • Utilize knowledge of SNHS and other Seneca Nation resources and departments.
  • Monitor patient progress and goals set by the Clinical Care Coordinator, Care Collaboration Supervisor, or other care providers.
  • Make referrals, coordinate care, act as a liaison with internal or external agencies, and advocate for patients by explaining entitlements and assisting with paperwork.
  • Accurately document patient case information, including notes, forms, and other pertinent details.
  • Schedule appointments, obtain referrals or orders, and gather significant health-related data.
  • Assist patients in creating appointment calendars, setting up cell phones, and finding resources for housing.
  • Assist patients in crisis by prioritizing and expediting linkage to necessary services and resources.
  • Assist patients with understanding and completing various paperwork.
  • May transport and accompany patients to meetings or health-related appointments with prior supervisor and director approval.
  • Complete monthly and other required reports by established deadlines.
  • Promote cooperative staff efforts and assist coworkers and other units to provide quality patient services.
  • Perform job functions with minimum supervision, keeping the supervisor updated on cases and any arising difficulties.
  • Exhibit sensitivity to community and cultural needs when performing job functions.
  • Maintain patient confidentiality and HIPAA standards.
  • Travel to both territories may be required.
  • Uphold and promote the value and integrity of the Seneca Nation Health System’s mission statement.
  • Provide care through a trauma-informed lens, incorporating principles of safety, trustworthiness, transparency, collaboration, empowerment, and cultural sensitivity.
  • Participate in and complete all mandatory staff meetings, trainings, and in-services.
  • Follow all policies and procedures of the department, SNHS, and Seneca Nation.

Requirements

  • Associates Degree in Human Services or related field required.
  • Two years’ experience working in a Human Services agency required.
  • A combination of education and experience in case management which assures ability and knowledge to perform duties may be acceptable.
  • Valid NYS driver’s license required.
  • Experience working in a mental health facility, hospital, medical center, outpatient health care setting, or Indian Health Service (IHS)/tribal health, preferred.

Skills

  • Understanding the practices and procedures of social casework, and the factors affecting disease and disability.
  • Excellent oral and written communication skills.
  • Ability to multitask, keeping organized with strong time management skills.
  • General working knowledge of computer programs and software.
