About the Role

The London HART Hub is a low barrier recovery-oriented space supporting individuals experiencing homelessness with mental health and addiction needs. Counsellors will support clients by developing therapeutic relationships, providing addiction treatment planning, increasing client motivation, and offering counselling to improve bio-psycho-social-spiritual health.

Responsibilities

• Provide Intake Services
• Administer and interpret screening tools to determine program eligibility and level of care
• Respond effectively to individuals/groups/communities in crisis
• Provide crisis intervention and de-escalation
• Adapt services to clients’ social and cultural needs and provide accessible communication options
• Assess client transportation needs and provide travel assistance
• Utilize standardized provincial assessment tools to determine client suitability for services and level of care
• Adhere to quality assurance measures for provincial standardized assessment tools
• Provide clinical interpretation of assessment tools and utilize motivational interviewing for client-centred treatment plans
• Provide re-assessment on an ongoing basis to assess treatment progression and adjust treatment plans
• Develop and maintain therapeutic relationships with clients
• Increase client motivation using a Stages of Change paradigm and motivational interviewing
• Apply a comprehensive range of evidence-informed counselling styles and techniques
• Use best practice, promising practices, and evidence-informed models and treatment modalities
• Develop a formalized treatment plan including case management, goal setting, counselling, referrals, advocacy, and education
• Evaluate and update treatment plans
• Match clients with the most appropriate available services
• Manage and meet targets for a clinical caseload and maintain direct service requirements
• Participate in the facilitation of community treatment groups
• Facilitate successful service navigation for clients through empowerment and/or advocacy
• Participate and lead client-centred case conferences
• Coordinate services for individuals involved in the social justice system
• Provide support regarding addiction treatment, including understanding of addiction types, treatment continuum, and continuum of use
• Provide rapid connection to housing stability resources
• Provide services in person, by phone, or other communication means regarding addiction and mental health services and supports
• Educate clients and concerned others regarding coping techniques and strategies
• Deliver effective outreach services targeting prevention, health promotion, early intervention, treatment, and/or after care
• Engage in community development, community awareness, education, and outreach
• Actively participate in clinical supervision
• Actively participate in administrative supervision
• Provide peer support

Requirements

• College Diploma or University Degree in a related field (e.g., BSW, post-graduate diploma in addictions or equivalent)
• Registration with a professional college (e.g., OCSWSSW, CNO, CRPO, CPSO, COTO, CPO)
• Valid Driver’s License: minimum G2 class required
• Transportation of clients in personal vehicle required; two million-dollar ($2,000,000.00) liability insurance required
• Current, clear Police Vulnerable Sector Check (PVSC)
• Demonstrated ability to attend work regularly

Skills

• ASIST Training
• Non-Violent Crisis Intervention Training
• Clinical work experience in substance use treatment including intake services
• Experience working with individuals with no fixed address
• Experience in crisis intervention/management and crisis resolution
• Experience with, and detailed knowledge of, mild, moderate & serious mental illnesses
• Knowledge of Psycho-social Rehabilitation
• Extensive knowledge of the support/treatment services network in London and regional services
• Knowledge of the legal system
• Knowledge and understanding of philosophies, practices, models and frameworks associated with Ontario’s London Hub plan
• Bilingual (both official languages, preferred)

• Full-Time
• Contract (Ending: Approximately 6 months)

• Grid progression based on seniority up to Level 8

Education Level

• College Diploma or University Degree in a related field

Salary/Compensations

• Starting rate: $27.14 per hour (Level 1 – Direct Service)
• Wage differential: $30.01 per hour for Level 1 – Direct Service, Regulated
• Non-regulated: $34.41 per hour
• Regulated: $37.26 per hour

• Immediately eligible for optional enrollment in the Healthcare of Ontario Pension Plan (HOOPP)
