About the Role
Episcopal Community Services (ECS) provides meaningful, mission-driven work supporting participants facing homelessness, behavioral health conditions, substance use challenges, and other difficult life circumstances. This role requires sound judgment, adherence to safety procedures, and prompt incident reporting. It is a fully onsite, client-facing position with direct service delivery.
Responsibilities
- Perform intake assessment of assigned residents, including health status, psychosocial history, financial status, substance use history, and housing history.
- Conduct regular outreach to residents and respond to incidents or documentation indicating a resident may be in crisis, need intervention, or may be jeopardizing their housing.
- Develop and implement individualized Service Plans based on resident goals, needs, and preferences.
- Provide referrals to internal or external resources for substance use, vocational training, legal services, immigration services, health services, housing, and money management.
- Assess resident eligibility for benefits and assist with applications and enrollment.
- Assist residents with budgeting and voluntary money management plans.
- Advocate on behalf of residents with Property Management to prevent eviction.
- Assist with conflict resolution to ensure a comfortable living environment.
- Participate in Wellbeing Checks and Emergency Safety Checks.
- Develop and facilitate at least one weekly therapeutic or psycho-educational group.
- Assist with organizing and executing program events and community-building activities.
- Maintain up-to-date, confidential, and professional documentation in resident files.
- Complete all necessary paperwork and administrative requirements within the required timeframe.
- Assist residents with planned exits and discharge planning, including referrals for necessary services.
- Assist in developing and implementing new programs, groups, and activities.
- Provide back-up coverage for other case managers as directed.
- Share information and new resources with residents and staff.
- Follow and implement all applicable policies and procedures of Episcopal Community Services.
- Establish and maintain positive working relationships with employees, volunteers, residents, and outside agencies.
- Attend all assigned meetings.
- Perform other duties as assigned.
Requirements
- High School Diploma + 4 years’ experience OR AA degree + 3 years’ experience OR CHW Certificate/CADC + 2-4 years’ experience OR BA/BS degree + 2-4 years’ experience OR MA/MS degree + 1 year experience (for Case Manager).
- MA/MS + at least 2 years’ experience OR BA/BS + at least 4 years’ experience OR CHW Certificate/CADC + 4 years’ experience (for Senior Case Manager).
- Knowledge of local social services and community resources for underserved populations and severely low-income individuals and families.
- Patience, compassion, and empathy towards participants.
- Excellent charting, recordkeeping, and professional writing skills.
- Ability to quickly assess and respond to various situations, including medical emergencies, psychiatric emergencies, conflict resolution, de-escalation, and other crises.
- Computer skills and moderate experience with Microsoft Outlook, Word, and other MS Office applications.
- Strong interpersonal skills, a team approach, and effective oral and written communication.
- Demonstrated cultural competency in working with diverse, underserved populations.
- Ability to maintain professional, respectful, honest, and calm interactions.
- Ability to work as part of a multi-disciplinary team.
- Comfortable facilitating groups and hosting events and activities.
- Commitment to the mission and values of Episcopal Community Services.
- Must secure finger image screening and annual TB screening.
- Must be able to meet physical requirements including long periods at a desk/computer, travel to various sites, and lifting up to 20 lbs.
