About the Role

The Community Habilitation Coordinator supports individuals with intellectual and developmental disabilities in developing independence and community integration skills. This role ensures services are person-centered, compliant with OPWDD regulations, and aligned with individual Life Plans.

Responsibilities

  • Establishes and maintains good working relationships with other service delivery agencies and IBERO programs.
  • Monitors service delivery to ensure goals and outcomes are addressed.
  • Assures all procedures are implemented as required by Federal, State, Agency and Program regulations, policies, and procedures.
  • Meets monthly with immediate supervisor to report issues, review past weeks, and plan for the following month.
  • Attends formal and informal training sessions.
  • Researches and links clients with appropriate services.
  • Completes and updates a written individualized service plan for each participant every six months.
  • Submits monthly reports on activities and related duties to the Director.
  • Develops individual progress towards goals as included in the individual service plan.
  • Provides advocacy and support services to individuals and their families.
  • Schedules and supervises Community Habilitation staff.
  • Completes individual satisfaction surveys to receive feedback on services.
  • Ensures staff are trained and follow service plans and documentation requirements.
  • Addresses staff performance concerns promptly and professionally.
  • Completes and updates written documentation monthly.
  • Is available to work outside of regular scheduled work hours as needed.
  • Monitors service quality and ensures compliance with health and safety standards.
  • Conducts field visits and observations of staff providing services.
  • Identifies areas for improvement and implements corrective actions.
  • Assists with staff scheduling and coverage as needed.
  • Tracks service utilization and ensures authorized hours are met.
  • Performs other duties assigned by supervisor.

Requirements

  • Must possess a valid NYS driver's license and meet Ibero's Vehicle Operator Requirements.
  • Knowledge of OPWDD regulations and person-centered planning.
  • Strong organizational, communication, and leadership skills.
  • Ability to work independently and collaboratively.
  • A combination of training and experience other than the specified, if judged to be adequate for the job, may be accepted by the President and CEO.
