About the Role

As a Complex Care Coordinator, you will coordinate care for individuals receiving palliative care in diverse and sometimes complex community settings. You will advocate for patients, upholding human dignity and providing support for them to live as actively as possible until the end of their days and enjoy optimal quality of life. You will play a key role in delivering interconnected, accessible, and patient-centered care, supported by a team of over 9,000 regulated health professionals and other professionals. You will help our patients achieve better health at home and benefit from our support for professional development, personal well-being, and work-life balance.

Responsibilities

Collaborate with patients and families to assess care needs, determine service eligibility, and develop individualized care and service plans for individuals with complex needs.
Connect patients with community service providers to ensure patient safety in their homes, prioritizing the prevention of hospitalizations and emergency department visits.
Coordinate and monitor the implementation of care plans.
Build a helping relationship with patients and families.
Act as an expert resource for colleagues and external partners regarding the palliative care needs of patients.
Align patient needs and choices with available resources, respecting patient values and preferences.

Requirements

Current registration with the applicable regulatory body as a Registered Nurse (RN, BScN).
Minimum of one year of experience in community health or a related field.
Palliative care training/certification (e.g., Fundamentals of Hospice Palliative Care, LEAP, CAPCE, etc.).
Knowledge of EDITH protocol, symptom management intervention kits, DNR confirmation documents, and Palliative Performance Scale (PPS).
Familiarity with common trajectories and prognoses for life-limiting illnesses.
Understanding of pain and symptom management needs for patients receiving palliative care.
Strong clinical background and a patient-centered approach.
Knowledge of the healthcare delivery system and community resources.
Natural ability to motivate people, with leadership and advocacy qualities.
Excellent interpersonal, communication, assessment, problem-solving, and decision-making skills.
Time management, prioritization, and organizational skills, with the ability to work independently and collaboratively in a fast-paced, multidisciplinary environment.
Established ability to accurately complete required documentation, reports, and forms.
Valid driver's license and access to a reliable vehicle.
Proficiency in the Windows environment.
Mandatory COVID-19 vaccination policy; proof of vaccination required prior to start date.
