About the Role

Access Alliance Multicultural Health and Community Services (AAMHCS) is seeking an individual to work within the Open Door Program (ODP), an outreach-based health initiative focused on improving health outcomes for vulnerable populations. This role involves increasing access to health education and services, providing preventative care, and facilitating navigation to primary care, mental health, addiction services, and community support.

Responsibilities

Plan, facilitate, and implement information sessions, workshops, and group programs related to chronic disease prevention, management, and other health topics.
Support individuals with developing preventive care plans.
Assist with the development of health information materials and resources, and displays based on ODP communication strategies.
Identify, compile, and maintain lists of culturally and linguistically appropriate community services, programs, and resources for ODP use.
Refer community members externally as appropriate.
Support the planning, implementation, and evaluation of community engagement programs/initiatives in accordance with project goals.
Assist with program volunteer recruitment, orientation, and training, as needed.
Assist in the promotion of programs, services, and special events.
Develop and maintain both informal and formal community networks and partnerships.
Work effectively with community partners to implement collaborative programs and initiatives, and support capacity building.
In collaboration with the ODP team, develop and roll out various outreach strategies in the community linked to identified needs.
Develop an annual work plan in conjunction with your supervisor, identifying action plans, learning goals, and professional development needs.
Refer community members internally (and externally) as appropriate.
Assist in program data collection, monitoring, evaluation, and funder reporting.
Submit program statistics and reports on a timely basis.
Manage all expenditures and reporting as per organizational policies.
Participate in, support, and actively contribute to all organizational planning, evaluation, systems, and structures.
Assist in other duties as assigned that are reasonable within the scope of the job.

Requirements

An undergraduate degree in Health Sciences, Social Work, Education OR a combination of relevant education and work experience.
Minimum 3 years of front-line experience in community-based work doing outreach, health education and group facilitation.

Skills

Good knowledge of health promotion, health education and health issues affecting newcomer communities.
Experienced in running programs and activities, organizing special events, coordinating engagement of other staff, volunteers, students and community members.
Experience working with low-income, multi-racial, multi-lingual communities and interdisciplinary teams.
Excellent interpersonal skills with colleagues and community members.
Excellent communication skills, both written and verbal in English.
Demonstrated flexibility and responsiveness to community needs.
Excellent administrative, time-management and problem-solving skills.
Experience coordinating volunteers.
Second language relevant to the populations served by the ODP.
