About the Role
Seeking a compassionate, adaptable, and experienced Social Worker to join a multidisciplinary team in a casual capacity. This role involves supporting patients and their families through hospitalization and discharge, coordinating safe transitions home, and liaising with care teams and families.
Responsibilities
- Lead and coordinate the safe transition of patients back home.
- Communicate daily with the care team about new admissions and upcoming discharges.
- Recommend services to meet patients' healthcare needs.
- Liaise with patient families.
- Initiate conferences and action plans.
Requirements
- A Social Work degree from a recognised university.
- Eligibility for membership with the Australian Association of Social Workers (AASW).
- Recent experience in discharge planning within an acute care setting.
- Strong communication skills, with the ability to engage professionally and compassionately with patients, families, visitors and colleagues.
- A collaborative and adaptable approach, with the ability to work effectively both independently and as part of a multidisciplinary team.
- An empathetic, organised and detail-oriented work style.
