About the Role

Our Amplar Preventative Health team helps Australians improve their health, prevent avoidable hospital admissions and achieve better long-term outcomes. We have an exciting opportunity for a Cardiac Rehab Program Clinician to join our national Heart Health at Home program. Using your cardiac rehabilitation expertise, you'll support participants recently discharged from hospital following a cardiac event. Managing your own caseload, you'll deliver evidence-based coaching, education and care planning via phone/video, helping participants build confidence, improve heart health and reduce the risk of future events.

Responsibilities

Delivering virtual cardiac rehabilitation coaching and education.
Conducting assessments and developing personalised care plans.
Monitoring participant progress and managing clinical risks.
Supporting participants to engage with the Cardihab app to support their rehab.
Communicating with GPs, specialists and other healthcare providers to coordinate care.

Requirements

At least 5 years professional clinical experience as either a Registered Nurse, Exercise Physiologist or Physiotherapist with current registration with AHPRA or ESSA.
2+ years’ experience delivering cardiac rehabilitation (phase 2) within the past 5 years.
Previous health coaching experience and understanding of health behaviour change methodology.
Experience in acute and primary health care and care planning.
Solid understanding of cardiac rehab programs, cardiac disease, and evidence-based practice.
