About the Role
Our Amplar Preventative Health team helps Australians improve their health, prevent avoidable hospital admissions and achieve better long-term outcomes. We have an exciting opportunity for a Cardiac Rehab Program Clinician to join our national Heart Health at Home program. Using your cardiac rehabilitation expertise, you'll support participants recently discharged from hospital following a cardiac event. Managing your own caseload, you'll deliver evidence-based coaching, education and care planning via phone/video, helping participants build confidence, improve heart health and reduce the risk of future events.
Responsibilities
- Delivering virtual cardiac rehabilitation coaching and education.
- Conducting assessments and developing personalised care plans.
- Monitoring participant progress and managing clinical risks.
- Supporting participants to engage with the Cardihab app to support their rehab.
- Communicating with GPs, specialists and other healthcare providers to coordinate care.
Requirements
- At least 5 years professional clinical experience as either a Registered Nurse, Exercise Physiologist or Physiotherapist with current registration with AHPRA or ESSA.
- 2+ years’ experience delivering cardiac rehabilitation (phase 2) within the past 5 years.
- Previous health coaching experience and understanding of health behaviour change methodology.
- Experience in acute and primary health care and care planning.
- Solid understanding of cardiac rehab programs, cardiac disease, and evidence-based practice.
