About the Role
Heartline Staffing, in partnership with Purple Cow Recruiting, is hiring a Licensed Mental Health Counselor (LMHC) for Syracuse Community Health in Syracuse, New York. This role involves providing outpatient behavioral health services within an integrated care model.
Responsibilities
- Provide outpatient behavioral health services through individual, family, and group counseling.
- Complete behavioral health assessments, establish diagnoses, and develop individualized treatment plans.
- Monitor patient progress and modify treatment plans based on clinical outcomes.
- Collaborate with primary care providers, psychiatric providers, and interdisciplinary team members to deliver integrated care.
- Provide crisis intervention, patient advocacy, and behavioral health education.
- Maintain timely and accurate documentation within the electronic medical record.
Requirements
- Active Licensed Mental Health Counselor (LMHC) license in New York.
- Experience providing individual, family, and group counseling.
- Ability to perform behavioral health assessments and develop treatment plans.
- Strong crisis intervention and patient advocacy skills.
- Experience collaborating within interdisciplinary healthcare teams.
- Excellent written and verbal communication skills.
- Electronic medical record documentation experience.
