About the Role
Provide outpatient behavioral health services for children, adolescents, and adults experiencing mental health or substance use disorders. Deliver individual, family, and group counseling utilizing evidence-based treatment approaches. Complete psychosocial assessments, treatment planning, crisis intervention, and ongoing care coordination. Collaborate with interdisciplinary providers to deliver comprehensive behavioral healthcare. Maintain accurate clinical documentation utilizing the electronic health record. Support a trauma-informed, recovery-oriented model of care focused on improving long-term patient outcomes.
Responsibilities
- Provide outpatient behavioral health services for children, adolescents, and adults experiencing mental health or substance use disorders.
- Deliver individual, family, and group counseling utilizing evidence-based treatment approaches.
- Complete psychosocial assessments, treatment planning, crisis intervention, and ongoing care coordination.
- Collaborate with Psychiatrists, Psychiatric Nurse Practitioners, case managers, and interdisciplinary providers to deliver comprehensive behavioral healthcare.
- Maintain accurate clinical documentation utilizing the electronic health record.
- Support a trauma-informed, recovery-oriented model of care focused on improving long-term patient outcomes.
Requirements
- Active Licensed Master Social Worker (LMSW) license in New York.
- Excellent verbal and written communication skills.
- Experience providing behavioral health counseling services.
- Ability to work with patients ages 5 and older.
- Comfortable providing individual, family, and group counseling.
- Experience collaborating within interdisciplinary healthcare teams.
- Electronic health record experience.
- Commitment to providing compassionate, patient-centered behavioral healthcare.
