About the Role

Rehab Without Walls is seeking an experienced SLP to provide functional rehabilitation in home and community settings. This role focuses on treating patients in real-life environments and offers an autonomous setting within a collaborative team.

Responsibilities

  • Perform evaluations and develop treatment plans for patients with speech, language, cognitive, and swallowing disorders.
  • Regularly re-assess treatment plan effectiveness, attend staffing meetings, and participate in family conferences.
  • Conduct discharge evaluations, recommend home and community safety measures, and provide referral and educational resources.
  • Communicate patient needs and progress to the treatment team, physician, patient, and family.
  • Collaborate with other disciplines to ensure coordinated care and improve patient outcomes.

Requirements

  • Valid Speech Language Pathology license in the state(s) of practice.
  • Current CPR Certification.
  • Ability to lift 50 pounds.
  • Ability to move, lift, or transfer patients, potentially up to 100 pounds, following safety procedures.
  • Duties require fine motor skills, visual acuity, and walking/standing for extended periods.
  • Additional physical requirements include frequent pushing/pulling, bending/stooping, reaching, kneeling, and positioning.
  • A health screen or examination may be required to verify the employee's physical capability to perform assigned duties with or without reasonable accommodations.

Skills

  • Effective and professional verbal and written communication.
  • Knowledge of rehabilitation techniques related to complex neurological injury preferred.
