About the Role
Seeking an experienced Physical Therapist for a primary therapist role, managing clients from evaluation to discharge. This role focuses on providing functional rehabilitation in real-life settings like home, school, work, and the community, offering an autonomous yet collaborative team environment.
Responsibilities
- Perform evaluations and develop treatment plans.
- Regularly re-assess treatment plan effectiveness.
- Attend staffing meetings and family conferences.
- Perform discharge evaluations and make recommendations for home and community safety.
- Provide referral and education resources.
- Communicate patient needs and progress to the treatment team, physician, person receiving services, and family members.
- Collaborate with other disciplines to ensure coordination of care and enhance patient outcomes.
Requirements
- Minimum of bachelor’s degree in Physical Therapy from an accredited program.
- Current, unrestricted license as a Physical Therapist by state.
- Current Basic Life Support or Cardiopulmonary Resuscitation certification.
- A minimum of one year’s work experience as a physical therapist preferred.
- Demonstrates knowledge of rehabilitation techniques related to complex neurological injury.
- Effective and professional verbal and written communication skills.
- Ability to lift 50 pounds.
- Ability to move, lift, or transfer patients, potentially up to 100 pounds following safety procedures.
- Duties require fine motor skills, visual acuity, and walking/standing for extended periods.
- Requires frequent pushing/pulling, bending/stooping, reaching, kneeling, and positioning.
- May require a health screen or examination prior to assignment and periodically thereafter.
