About the Role
The Home Care Worker provides direct services to frail elders in their homes, aiming to maintain and increase their ability to live safely and comfortably. This role is part of a health care team that transforms lives and enhances communities.
Responsibilities
- Provides personal care and assistance in people’s place of residence.
- Performs homemaker services.
- Assists participants with personal care, including colostomy care, bathing, peri-care, and care of mouth, skin, and hair.
- Assists participants with eating.
- Assists with toileting, including the use of a bedpan.
- Assists with ambulation.
- Performs participant laundry.
- Performs cue glucose tests and BP checks.
- Collects urine, sputum, etc.
- Escorts and assists clients to, from, and at medical appointments.
- Provides behavioral/emotional supervision or support, including reality orientation or reminders for nursing/medical care and diet restrictions.
- Assists in centers or clinics.
- Assists with meal preparation, including special diets and set up for meals.
- Assists with special program/participant needs under supervision of professional staff.
- Keeps areas of patient care clean and safe.
- Regularly reports on participants’ status and progress to homecare administrative staff.
- Fulfills administrative requirements such as reporting, keeping statistical records, and attending staff conferences and meetings.
- May participate in the training and orientation of other staff, volunteers, etc.
- Actively participates in and encourages actions that promote good public relations.
- Works a flexible schedule, including some weekend days, as the program provides services seven days a week.
- Works in all districts of San Francisco.
Requirements
- At least one year of experience working with older adults (paid or unpaid capacity).
- Ability to comprehend complex written materials and verbal instructions.
- Ability to communicate and speak clearly in English.
- Must be able to lift, transfer, push/pull, maneuver, and reposition 35 lbs.
- Ability to reach, bend and walk.
- Ability to perform fine motor tasks with hands.
