About the Role

The Primary Care Nurse (PCN) provides high-quality, comprehensive nursing care to individuals and families. As part of a clinical team, the PCN enhances care safety, effectiveness, and efficiency by performing nursing responsibilities and delegating tasks to medical assistants. The role involves patient education on preventive, chronic, and episodic health issues, and coordinating care with primary care providers to improve patient health status.

Responsibilities

Support primary care of patients under a planned care model.
Collaborate with PCP to identify needs for preventive care, evidence-based chronic disease care, and care coordination.
Provide general and specific health care education.
Work with patients to identify and monitor self-management goals.
Collaborate care with other community-based providers.
Follow up on emergency room visits and hospital discharges.
Assist providers with exams, treatments, and procedures as needed.
Ensure safe, accurate, and clinically appropriate collection, storage, and processing of all biologic specimens.
Administer medications, including vaccines and immunizations, via various routes as ordered by a provider or under standing orders.
Adhere to CHC protocols regarding documentation of medication, route, dosage, and patient information.
Monitor patient response to medication and report/respond to adverse reactions.
Provide patient education relevant to administered medication.
Perform nursing activities such as wound care and catheterization as prescribed by a primary care provider.
Accurately document all aspects of patient care in the medical chart in a timely manner.
Schedule tests and referrals for specialty care as ordered by a provider.
Maintain strict infection control standards in accordance with CHCI protocols, universal precautions, OSHA, and CLIA standards.
Act as a resource for medical assistants regarding patient preparation, needs, and procedures.
Monitor and ensure follow-up of all abnormal Pap Smear results, STDs, lead screens, and other tests performed at CHCI.
Oversee the maintenance of supplies and equipment, ordering as necessary.
Maintain compliance with agency mandatory annual training.
Utilize electronic health records for timely, safe, and confidential storage and retrieval of patient information.

Maintains compliance with agency mandatory annual training (Patient Safety, Abuse, Age Based Competencies, EOC, Abuse trainings, etc.).
Oversees the maintenance of supplies and equipment, ordering supplies as necessary.
Monitors and ensures follow up of all abnormal Pap Smear results, STDs, lead screens and other tests and screens performed at CHCI.
Acts as a resource person for medical assistants regarding patient preparation, needs and procedures needed.
Maintains strict infection control standards in accord with CHCI protocols and universal precautions OSHA and CLIA standards.

Administers medications, including vaccines and immunizations, orally, subcutaneously, intramuscularly, intradermly or via nebulizer at the order of prescribing provider and under standing orders as established by CHCI.
Ensures safe, accurate, clinically appropriate collection, storage, and processing of all biologic specimens.
Assists providers with exams, treatments, and procedures as needed.
Follows up on emergency room visits and hospital discharges.
Collaborates care with other community based providers.
