About the Role

ASCEND Mental Wellness is seeking a Court Navigator to join the Outreach/LEAMHRS program within Supportive Residential Services. This role involves connecting with individuals to assess their needs for mental health, substance abuse services, and social determinants of health, facilitating their access to necessary resources.

Responsibilities

  • Receive referrals for individuals needing support with mental health, substance use, housing, legal, or basic needs.
  • Contact individuals to discuss services and initiate referral processes.
  • Maintain follow-up with individuals and perform warm handoffs to providers.
  • Collaborate with residential staff to identify pathways to permanent housing.
  • Participate in case reviews and staff meetings with behavioral health and criminal justice system stakeholders.
  • Collaborate with various community entities including psychiatric facilities, substance use facilities, medical centers, law enforcement, and social services.
  • Navigate multiple online platforms and document daily encounters.
  • Maintain a safe environment for clients and staff.
  • Communicate effectively with staff and supervisors.
  • Preserve human dignity and confidentiality of clients.

Requirements

  • At least 3 years of professional experience in both community mental health and criminal justice.
  • Must be of good character and background.
  • Must display an affinity for working with people with mental illness and/or co-occurring disorders.
  • Ability to work independently.
  • Good problem-solving skills.
  • Ability to work as part of a multi-disciplinary team.
  • Valid NYS driver's license acceptable to Agency insurance standards.

  • ASCEND works with the Glens Falls Police Department, Hudson Falls Police Department, Warren and Washington County Sheriff's Offices, Warren and Washington County Jails, Warren County DSS, and Warren and Washington County Probation Offices.
  • Individuals can be referred to through social services, care management and medical services based on need.
  • Assistance with obtaining health insurance and emergency housing are available.
