About the Role

The Occupational Therapist (OT) provides therapy services to individuals in the community within the framework of VON policies, procedures, standards, and quality and risk management. The OT collaborates with stakeholders and community services to deliver client-centered and evidence-based therapy.

Responsibilities

• Assess, plan, evaluate, and document client care plans using a client-centered and evidence-based approach.
• Assess clients' ability to function safely in their home/community environment, including Activities of Daily Living (ADLs), Instrumental ADLs, and equipment needs.
• Assess clients' cognitive ability to function independently in the home/community.
• Conduct holistic client assessments, considering mental health, social contacts, spiritual, and physical needs impacting independence.
• Develop client-centered care plans in collaboration with the client and healthcare team.
• Provide education and recommendations to clients regarding their therapy goals.
• Assist clients with funding sources for equipment and home/vehicle modifications.
• Monitor client outcomes and progression towards therapy goals.
• Modify care plans based on client outcomes.
• Document client outcomes and care plans in the electronic client chart according to guidelines.
• Communicate with funding agencies regarding client outcomes and status changes.
• Manage and maintain caseloads, responding to changes in volume.
• Complete and update electronic care plans with information from clients and healthcare/educational professionals.
• Utilize verbal and non-verbal communication skills effectively with clients.
• Demonstrate professional code of conduct and communication with the health team.
• Promote VON's goals and values as an integrated community care provider.
• Promote a safe and healthy workplace in accordance with the Occupational Health and Safety Act and VON Safety Management System.
• Abide by all VON policies, work practices, and confidentiality policies.
• Collaborate with the interdisciplinary team to meet client care needs and therapy goals.
• Work collaboratively with community services for safe transitions and goal achievement.

Requirements

• Must have a personal vehicle.
• Must possess a current driver's license and proof of vehicle insurance.
• Willingness to travel regularly.
• A current and original satisfactory Criminal Records Check is required.
• A Vulnerable Sector Search and/or Child Abuse Registry Check may be required.
• Must be able to wear Personal Protective Equipment (PPE).

• Masters in Occupational Therapy (or degree prepared occupational therapy education prior to 2008)
• Valid registration with the College of Occupational Therapists and associated liability insurance
• ADP Authorization Certificate is required
• Member with the provincial Society of Occupational Therapist is preferred
