About the Role
The Tenant Services Coordinator provides direct care services and referrals to single, near-elderly, adult households at the 315 West 94th Affordable Housing Services Program. This role works with the Assistant Site Director to promote continued stable living for households on their caseload.
Responsibilities
- Coordinate day-to-day on-site services for approximately 20-25 households, ensuring services align with RiseBoro’s mission.
- Complete needs assessments and develop tenant-centered support plans, reviewing and documenting progress.
- Provide case management, assistance, and advocacy, including referrals to community services.
- Ensure tenants maintain housing subsidies and benefits.
- Assist tenants in obtaining and maintaining government benefits through advocacy.
- Facilitate tenant engagement through educational workshops and social activities.
- Educate residents on available on-site services and community resources.
- Prepare and maintain up-to-date records and resident files, including personal information, eligibility, services provided, and correspondence.
- Conduct monitoring services through visits and telephone calls, and periodic inspections with property management.
- Collaborate with community partners to ensure residents can age in place, coordinating home care and discharge planning.
- Communicate with property management and maintenance staff regarding resident issues and incidents.
- Participate in case conferences and coordinate with program staff for tenant and programmatic success.
- Monitor recreational activities such as art, yoga, and gardening sessions.
- Additional responsibilities may be added as the position evolves.
Requirements
- Team player willing to work with a diverse population.
- Ability to problem-solve and work independently.
- Experience working with the homeless population and/or seniors and older adults with comorbid substance abuse, mental health, medical health, or related diagnoses.
- Six months of human service experience preferred.
- Flexible to accommodate weekend and evening hours to meet tenant needs.
- Commitment to RiseBoro’s mission.
