About the Role

As a Care Partner, you will play a key role in helping older Australians remain independent, connected, and living well in their own homes. You will deliver person-centred care management that supports each customer's goals, preferences, and wellbeing by working closely with customers, families, carers, and health professionals.

Responsibilities

  • Develop, implement, and review personalised care plans that promote independence, wellbeing, and customer choice.
  • Coordinate services and manage customer budgets within approved funding parameters.
  • Build trusted relationships with customers, carers, families, and health professionals.
  • Monitor customer wellbeing, identify changing needs, and respond proactively to potential risks.
  • Apply wellness and reablement principles to empower customers to maintain their independence.
  • Support customers to make informed decisions and navigate aged care services with confidence.
  • Maintain accurate documentation, care notes, and records in line with legislative and organisational requirements.
  • Promote safe, high-quality, culturally responsive care that respects each customer's individual needs, values, and preferences.

Requirements

  • Certificate III in Individual Support (Ageing), Certificate IV in Aged Care, or an equivalent qualification.
  • Minimum 12 months experience in aged care, disability, community services, or a related sector.
  • Excellent communication and relationship-building skills with customers, families, carers, and health professionals.
  • Strong organisational and time management skills with the ability to manage competing priorities.
  • Confidence using client management systems and other digital platforms.
  • Ability to work independently while contributing positively to a collaborative team environment.
  • Proof of COVID-19 vaccination status (or as required by legislation and WMQ policy).
  • Relevant probity checks required by legislation and WMQ Limited policy.
  • Current Driver’s Licence and willingness to travel for work.

Salary/Compensations

  • Salary Packaging up to $15,900 annually for everyday living expenses
