About the Role
VHA's Social Worker provides consultation, assessment, and planned interventions for diverse client groups across Southern Ontario and the Capital region. The role involves working within a continuous quality improvement framework to promote and maintain good public relations with clients, customers, and VHA staff.
Responsibilities
- Assesses psychosocial needs of clients and their families to identify factors affecting their response to illness, need for care, and ability to adjust.
- Counsels clients and family members concerning long-range planning, including evaluation of home situations for long-term care.
- Assists families in developing in-home care systems or exploring alternatives to in-home care, including arranging placement when indicated.
- Provides short-term therapy to clients and family members.
- Delivers crisis intervention or problem-solving for high-risk indicators affecting clients' and dependents' mental and physical health.
- Ensures coordinated care delivery through communication with the client's team regarding intervention plans and schedules.
- Documents all assessment findings, treatment plans, progress, and discharge summaries as required.
Requirements
- Master of Social Work Degree from a recognized university.
- Registered with the Ontario College of Social Workers and Social Service Workers.
- At least two years of clinical experience, preferably in the community.
- Specialized courses to support areas of clinical expertise.
- Clinical expertise to provide a variety of social work interventions.
- Knowledge of community-care requirements and available resources.
- Familiarity with the routine use of valid and reliable outcome measures to support clinical interventions.
