About the Role
Provides case management services for Rawhide clients, coordinating the interdisciplinary plan of care from admission to discharge and aftercare. Acts as a liaison between clients, families, placing agencies, and the Rawhide treatment team to ensure prompt and effective care. Oversees alumni services, needs, requests, and scholarships.
Responsibilities
- Ensures the therapeutic needs of clients and families are met by working collaboratively with the therapist and living unit personnel.
- Provides crisis support by maintaining availability to residents and living unit staff when called upon.
- Acts as a liaison to placing agencies and families by establishing and maintaining mutual understanding, cooperation, and collaboration.
- Provides case management to support residents by developing treatment plans and aftercare.
- Maintains Rawhide's stability, reputation, and mission by ensuring case management aligns with goals, philosophies, and state licensing regulations.
- Provides aftercare services by advising clients and alumni, assessing needs, and ensuring clients have resources for community transition.
- Maintains historical reference by following procedures for retention, protection, retrieval, transfer, and disposal of records.
- Maintains professional and technical knowledge by attending educational workshops, reviewing professional publications, and keeping informed of state licensing rules.
- Protects the credibility of the Rawhide mission by complying with the Rawhide standard of conduct and setting a positive example.
- Contributes to team effort by performing other duties as directed or assigned by supervisor.
- Provides care management services for Rawhide clients to ensure proper care is followed both at intake, during and aftercare.
- Serves as a liaison to placing agencies and families.
- Ensures therapeutic needs of the client are being met.
Requirements
- Bachelor’s degree in a social or behavioral science field with two years of related work experience, or a Master’s degree in a social or behavioral science field.
- Strong interpersonal, organizational, and administrative skills.
- Excellent written and verbal communication skills.
- Sound decision-making skills.
