About the Role

The Clinical Pharmacist – Inpatient PRN supports the inpatient pharmacy team at Medina Hospital, providing comprehensive clinical and operational pharmacy services across a community hospital with a strong focus on orthopedic care. Working collaboratively with pharmacists, clinical specialists, physicians, nurses, and other members of the interdisciplinary care team, this role helps ensure safe, effective, and evidence-based medication therapy.

Responsibilities

Verifying and processing medication orders
Performing clinical chart reviews and medication reconciliation
Entering and clarifying medication orders within the electronic health record
Responding to medication-related questions from caregivers throughout the hospital
Evaluating medication appropriateness
Resolving formulary and therapeutic issues
Partnering with the care team to optimize patient outcomes
Supporting efficient inpatient pharmacy operations
Provide safe, efficient, patient-centered and evidence-based medication services
Verify medication orders and/or dispense medications accurately according to state and federal regulations
Document services in a complete, accurate and timely manner
Ensure all pharmacy operations comply with national accreditation standards
Maintain professional interactions with patients to facilitate successful patient experiences
Educate and train students, residents, other caregivers and/or patients

Requirements

PharmD or Bachelor’s of Science in Pharmacy from an accredited college of Pharmacy
Current State licensure as a Registered Pharmacist (RPh) or eligible for reciprocation in current state within 90 days of hire
Current Basic Life Support (BLS) Certification through the American Heart Association (AHA) or American Red Cross
Completion of a Board of Pharmacy approved course in the administration of immunizations or certification within 90 days of hire for individuals administering immunizations
Ability to perform the job while standing and walking for prolonged times, approximately 75% of shift and continuously for extended periods of time
Manual dexterity sufficient to perform computer and dispensing functions of the job
Ability to lift up to 30 lbs
Position often includes reaching, stretching, stooping, climbing steps and sitting
Must have normal or corrected vision
Must have ability to clearly communicate verbally by phone or in person
Follows standard precautions using personal protective equipment as required
Required to comply with the influenza prevention program, which will include obtaining an influenza vaccination on an annual basis or obtaining an approved exemption
