About the Role
Registered Practical Nurses (RPNs) are sought for the Hospital Partnership Program to provide and coordinate home health nursing services. This role focuses on providing care to clients for 90-120 days post-hospital discharge, ensuring safe and personalized care in the client's home. Nurses will conduct comprehensive assessments and deliver various services, coordinating care with hospital partners and VHA specialists.
Responsibilities
- Assess, monitor, and make care planning and care coordination decisions.
- Evaluate care plan interventions through ongoing reassessment and evaluation.
- Ensure quality patient care by identifying and acting on factors affecting practice autonomy and care delivery.
- Demonstrate professionalism, leadership, judgment, and accountability in client and family interactions.
- Support safe client care through continuous learning and improvement.
- Inform the nursing supervisor of client activities, concerns, or changes in condition and submit documentation.
- Maintain documentation standards according to the College of Nurses of Ontario.
- Participate in a standby/on-call rotation for calls and response.
- Participate in team meetings as required.
Requirements
- Registered and in good standing with the College of Nurses of Ontario (CNO).
- RPN diploma from a recognized university or college.
- Valid First Aid or CPR Certification.
- A valid Ontario driver’s license (G2 or G License) and a reliable vehicle for traveling are required.
- Demonstrate effective communication (written and verbal) and documentation skills.
- Demonstrate reliable attendance, punctuality, and availability.
- Commitment to lifelong learning, professional growth, and critical thinking for client care.

Salary/Compensations
- $26.60 - $32.54 /visit (placement on salary range as per Collective Agreement) + $5.00 Top Up Premium Per Visit
- Estimated annual earning potential: $70,000–$120,000. Actual earnings may vary based on availability and service volumes.
