About the Role

The Resident and Family Coordinator plays a critical role in ensuring a smooth and supportive transition for new residents into the home. This role manages the admissions process, provides ongoing psycho-social support, and collaborates with interdisciplinary teams to enhance resident well-being. The individual acts as a primary liaison between residents, their families, and the home to ensure continuity of care and overall satisfaction.

Responsibilities

  • Oversee the management of applications for interdisciplinary review, ensuring timely scheduling and facilitation of home tours.
  • Communicate admission decisions in writing to applicants and substitute decision makers (SDMs).
  • Help prepare residents and their SDMs before and at arrival to ensure they are comfortable and understanding of the move processes.
  • Personalize resident rooms and engage with new residents and their SDMs to ensure a smooth transition and understand potential issues.
  • Coordinate and facilitate annual care conferences, collaborating with a multidisciplinary team to provide comprehensive support for residents.
  • Deliver psycho-social support to residents, working closely with the palliative care team to ensure comfort for residents in palliative stages.
  • Provide support to families throughout the discharge process.
  • Act as a primary point of contact for residents and their families, fostering communication through one-on-one interactions, surveys, and councils.
  • Relay any concerns or issues to management for prompt resolution.
  • Collaborate with the home to uphold resident care standards, ensuring that residents' needs are identified and addressed effectively.
  • Maintain up-to-date care assessments to guarantee quality of care.

Requirements

  • Registered Social Service Worker (RSSW) designation.
  • Membership in the Ontario College of Social Workers and Social Service Workers (OCSWSSW).
  • Experience in a healthcare or long-term care setting is preferred.
  • Previous experience in admission coordination or case management is an asset.
  • Excellent interpersonal and communication skills.
  • Strong organizational abilities.
  • An empathetic approach to care.
  • Ability to handle sensitive situations with tact and professionalism.
  • Understanding of the long-term care sector.
  • Knowledge of relevant legislation and standards (e.g., Long-Term Care Homes Act).
  • Comfort with facilitating care conferences.
  • Experience in a leadership or coordination role within an interdisciplinary team.
