About the Role
This role promotes independent living skills and stability for high-risk individuals by providing holistic, culturally sensitive case management. The position involves linking clients to service systems, coordinating care, and monitoring progress to encourage community integration.
Responsibilities
- Provide one-on-one support to service recipients
- Maintain working relationships with client families
- Report pertinent information regarding illnesses, accidents, or staff difficulties to supervisors
- Coordinate all services received by the client
- Develop and update individualized care management plans (IAP)
- Link individuals to necessary services and supports
- Advocate for client access to services in the least restrictive setting
- Maintain accurate written progress notes per agency guidelines
- Develop and maintain community and provider relationships
- Assist participants with academic, rehabilitation, social, employment, and health supports
- Coordinate treatment with external agencies
- Perform problem-solving functions to overcome obstacles
- Ensure resident records comply with federal, state, and agency regulations
- Conduct regular record-keeping reviews and audits
- Work within program budgets
- Monitor and maintain high standards of service delivery
- Collect statistical data
- Maintain client confidentiality
- Adhere to all agency policies and procedures
Requirements
- Valid driver’s license and acceptable driving record
- Ability to use a personal vehicle for client transport when agency vehicles are unavailable

Experience Level
- Two years of relevant experience with a Bachelor's degree
- Two years of relevant experience as a Registered Nurse
- One year of relevant experience with a Master's degree
