About the Role

As a Care Coordinator, you will determine patient eligibility and care needs, ensure access to and referral to community services, and collaborate with patients, caregivers, and healthcare professionals. You will play a key role in delivering interconnected, accessible, and patient-centered care, supported by a team of over 9,000 regulated health professionals and other professionals. Your mission will be to help our patients achieve better health at home, with support for professional development, personal well-being, and work-life balance.

Responsibilities

  • Assess care needs, determine service eligibility, and develop individualized care and service plans in collaboration with patients and families.
  • Connect patients with service providers.
  • Coordinate and monitor the implementation of care plans.
  • Build a helping relationship with patients and their families.
  • Align patient needs and choices with available resources, ensuring respect for patient values and preferences.

Requirements

  • Current member in good standing with the applicable regulatory body (e.g., College of Nurses of Ontario, College of Physiotherapists of Ontario, etc.).
  • Minimum of two years of experience in community health or a related field.
  • Knowledge of the healthcare delivery system and community resources.
  • Excellent interpersonal, communication, assessment, problem-solving, and decision-making skills.
  • Time management, prioritization, and organizational skills, with the ability to work independently and collaboratively in a fast-paced, multidisciplinary environment.
  • Proven ability to accurately complete required documentation, reports, and forms.
  • Valid driver's license and access to a reliable vehicle.
  • Proficiency in the Windows environment.
  • Mandatory COVID-19 vaccination policy; proof of vaccination required prior to start date.
