About the Role

Health Quest Home Care is seeking a Home Health Aide to assist patients with personal care, exercises, meal preparation, and medication prompts. This role requires identifying changes in patient condition and taking vital signs.

Responsibilities

  • Assists the patient with personal care activities including bathing, oral hygiene, feeding and dressing.
  • Assists the client with exercises, ambulation, and transfer activities, including the use of assistive devices such as canes, walkers, crutches, and lifts.
  • Independently assists the patient with meal preparation/cooking and/or feeding.
  • Must have working knowledge of special diets, diet restrictions, and techniques to maintain sanitary kitchen conditions to prevent food-borne illnesses.
  • Assists the patient with medication prompts to remind the patient to self-administer his/her medications.
  • Identifies changes in patient condition that require notification to the patient’s registered case manager.
  • Takes vital signs to include temperature, pulse, heart rate, respiratory rate.

Requirements

  • Active NYS Home Health Aide Certification.
  • Driver's License.
