Responsibilities
- Assist patients with personal care activities including bathing, oral hygiene, feeding, and dressing.
- Support clients with exercises, ambulation, and transfer activities using assistive devices like canes, walkers, crutches, and lifts.
- Prepare meals and assist with feeding while maintaining sanitary kitchen conditions.
- Provide medication prompts to remind patients to self-administer medications.
- Monitor and report changes in patient condition to the registered case manager.
- Measure and record vital signs including temperature, pulse, heart rate, and respiratory rate.
Requirements
- Active NYS Home Health Aide Certification
- Valid driver's license
