About the Role

As a Case Coordinator, you will provide personalised and responsive case coordination services to clients, helping them navigate complex systems and empowering them to reach their goals through compassionate, culturally safe, and trauma-informed support.

Responsibilities

  • Conduct thorough assessments to determine client eligibility
  • Work with clients to create and regularly update personalised case plans
  • Assist clients with referrals to appropriate community supports and resources
  • Ensure client safety by performing risk assessments and creating safety plans
  • Maintain accurate record keeping
  • Build strong relationships with clients and their families using strengths-based and culturally sensitive approaches

Requirements

  • Tertiary qualifications in Human Services or related discipline (minimum Certificate IV with relevant experience)
  • Case coordination experience, including developing and implementing case plans
  • Ability to perform comprehensive assessments in areas such as finances, health, housing, social needs, vocational skills, and education
  • Experience managing clients with complex physical and mental health issues
  • Working understanding of current casework practices for culturally and linguistically diverse clients
  • Successful completion of probity checks including National Criminal History Record Check and relevant state-based checks
