About the Role

Provide high-quality, affordable, and accessible healthcare to a diverse community as a member of a healthcare team in a School Based Health Center.

Responsibilities

• Provide primary health care services directly in a School Based Health Center, emphasizing accessibility, health promotion, illness prevention, and continuity of care.
• Provide primary care at other clinical sites, including primary care sites and mobile units during non-school hours or as assigned.
• Take complete medical, psychosocial, economic, relationship, and family status histories.
• Complete physical examinations and perform/order appropriate diagnostic tests.
• Administer vaccines according to accepted guidelines and medications as appropriate.
• Manage common acute and chronic illnesses, including minor trauma, episodic problems, and common chronic disorders.
• Discuss health concerns/diagnosis with the patient and collaboratively determine an appropriate plan of care.
• Demonstrate knowledge of normal growth and development, age-specific safety precautions, and age-appropriate anticipatory guidance.
• Refer complex and high-priority cases to the child’s primary physician or collaborating physician.
• Document all patient encounters completely and in a timely manner, including phone calls, prescriptions, and patient visits.
• Adhere to clinical practice guidelines adopted by the specialty group or an appropriate committee.
• Keep medical knowledge current through attendance at medical CME programs (at least 20 hours per year).
• Maintain compliance with clinical and regulatory standards, including medical record documentation per Joint Commission standards.
• Demonstrate appropriate interactions toward patients to help put them at ease.
• Participate in Performance Improvement activities through chart audits and development/implementation of practice standards.
• Actively support the implementation of the Patient Centered Medical Home model of care.
• Work collaboratively with the multidisciplinary team to provide treatment, ongoing follow-up, and evaluation of the plan of care.
• Assist school and health center staff willingly in difficult situations.
• Demonstrate constructive problem-solving with colleagues and staff.
• Support established policies and procedures of the School Based Health Center and contribute to policy formulation.
• Start patient schedules on time and complete work schedules including necessary record documentation.
• Complete required documentation for referrals and consultations.
• Return patient telephone calls within an appropriate timeframe.
• Perform other administrative tasks as assigned.
• Assist with on-call coverage as determined by leadership.
• Participate in promoting awareness, enrollment, and utilization of SBHC among students, staff, and families.
• Participate in community activities and research to promote health and prevent diseases.
• Supervise the clinical practice of medical/PA/NP students and residents.
• Act as a resource to staff and the community regarding current health care trends and specific health issues.
• Demonstrate excellence in both internal and external customer service.
• Understand and effectively communicate HIPAA compliance, corporate compliance, and client confidentiality.
• Ensure and/or remain in compliance with local, state, and federal regulations and accreditation standards.
• Adhere to the National Patient Safety Goals.
• Complete other duties as assigned.

Requirements

• Graduate of a registered approved Nurse Practitioner/Physician Assistant Program.
• Completion of required State/National Certification Programs.
• Current licensure and registration as a Registered Professional Nurse/Physician Assistant in the State of New York.
• Board Certification as a Nurse Practitioner/Physician Assistant.
• Current DEA license.
• Previous experience providing medical care within a pediatric/family medicine practice setting.
• Advanced Life Support (ALS) certification.
• Demonstrated competence in a multicultural medical setting.
• Demonstrated proficiency in Microsoft Excel, Windows, Outlook, Word.
