About the Role

The Rehab Field Supervisor ensures clinical supervision of staff and competent, effective, and safe rehabilitation services, including physical therapy, occupational therapy, and speech-language pathology. This role also assists with staff education, training, and development to maintain professional competence.

Responsibilities

  • Ensure knowledge of CHHA Regulatory guidelines per Center for Medicare and Medicaid, New York State Department of Health (DOH), and Joint Commission (JC).
  • Ensure knowledge of company policies and procedures related to clinical staff education and training.
  • Assist in conducting orientation and training for new staff on Extended Home Care’s policies and procedures.
  • Accompany new staff into the field post-orientation, conduct random field visits, and complete annual performance evaluations and field assessments.
  • Provide direct supervision of field staff, including on-site supervisory visits and chart reviews.
  • Evaluate and suggest revisions to the existing clinical documentation system.
  • Assist in screening and interviewing new personnel and make employment recommendations.
  • Assist in the orientation of new organization personnel.
  • Assist the Director of Rehab Services in planning and implementing in-service training and continuing education programs.
  • Participate in McKesson/Netsmart Software Upgrade, Testing, and Staff Education.
  • Participate in Quality Assurance Performance Improvement (QAPI) activities.
  • Participate in the Agency’s Emergency Preparedness Plan.

Requirements

  • A graduate of an approved program in physical, occupational, or speech therapy.
  • New York State License as a physical, occupational, or speech therapist.
  • Current registration from NYS Department of Education in physical, occupational, or speech therapy.
  • Minimum of three (3) years of work experience as a physical, occupational, or speech therapist in a medical setting.
  • Two years’ experience providing physical, occupational, or speech therapy in the home care setting.
  • Previous supervisory experience in the home care department is preferred.
  • Demonstrates leadership and strong interpersonal communication skills.

  • $115,000 – $125,000 per year
