About the Role

Episcopal Community Services (ECS) provides support to individuals experiencing homelessness, behavioral health conditions, and substance use challenges. This client-facing role involves direct service delivery, crisis intervention, and case management to help residents achieve stability and housing retention.

Responsibilities

  • Perform intake assessments including health, psychosocial, and housing history.
  • Conduct regular outreach to residents and respond to crisis situations.
  • Develop and implement individualized Service Plans based on resident goals.
  • Provide referrals to in-house and community resources for health, vocational, and legal services.
  • Assess eligibility for benefits and assist with enrollment applications.
  • Support residents with budgeting and voluntary money management plans.
  • Advocate for residents with Property Management to prevent eviction.
  • Facilitate conflict resolution to promote a respectful living environment.
  • Conduct regular Wellbeing and Emergency Safety Checks.
  • Develop and facilitate weekly therapeutic or psycho-educational groups.
  • Organize and lead community-building events and activities.
  • Maintain accurate, confidential, and professional resident documentation.
  • Complete administrative paperwork including Significant Encounters and service plans.
  • Assist with comprehensive discharge planning for planned program exits.
  • Collaborate with the Support Services Manager to develop new programs.
  • Provide back-up coverage for other case managers as needed.
  • Maintain positive relationships with employees, volunteers, and outside agencies.

Requirements

  • High School Diploma with 4 years of experience, or AA degree with 3 years of experience.
  • CHW Certificate or CADC with 2-4 years of experience.
  • BA/BS degree with 2-4 years of experience.
  • MA/MS degree with 1-2 years of experience.
  • Knowledge of local social services and community resources for underserved populations.
  • Ability to assess and respond to medical, psychiatric, and conflict-related crises.
  • Strong interpersonal, charting, recordkeeping, and professional writing skills.
  • Ability to work as part of a multi-disciplinary team.
  • Commitment to the mission and values of Episcopal Community Services.
  • Must secure finger image screening and annual TB screening.
  • Ability to lift up to 20 lbs and perform desk-based work.
  • Demonstrated cultural competency.
