About the Role
The Resident Services Coordinator provides outreach, case finding, assessments, and care planning to senior, disabled adults, and families within the scope of the Resident Services Program, addressing psychosocial, health, financial, recreational, nutritional, and legal needs.
Responsibilities
- Establishes and maintains relationships with residents, families, and neighborhood groups.
- Identifies and documents residents in need of services and monitors their health status.
- Serves as a liaison or advocate with outside social and health agencies for residents.
- Provides individual casework and case management, including assessment, care plan development, service coordination, and monitoring for frail or at-risk residents.
- Offers information and referral, crisis intervention, and counseling to individuals and groups.
- Maintains accurate and up-to-date client records and files.
- Identifies and resolves barriers to service provision.
- Facilitates and supports neighbor-to-neighbor, family, and volunteer caregiving, including managing volunteer programs.
- Provides residents with information on services, benefit programs, and resources.
- Identifies and develops training opportunities for resident groups and staff.
- Collaborates with the Health Education Coordinator to meet residents' holistic needs.
- Collects data to measure the impact of the WellElder program.
- Works cooperatively with housing managers or agency directors to address resident needs.
- Ensures completion of required documentation and statistics.
- Performs other duties as assigned.
Requirements
- Ability to relate appropriately and effectively to residents, families, staff, and community agency personnel.
- Ability to work independently and as part of an interdisciplinary team.
- Ability to maintain a positive, customer-focused approach and confidentiality.
- Ability to act with patience, tact, and courtesy under demanding conditions.
- Sensitivity and understanding of issues related to aging.
- Proficient with Microsoft Office suite (Word, Excel, Outlook).
- Physical ability to perform work requiring sitting, walking, stooping, bending, and lifting up to 35 pounds.
- California driver’s license, proof of insurance, and clean driving record.
- Ability to travel to multiple locations on a daily basis.
- Bilingual capacity in Cantonese or Mandarin preferred.

- Master's degree in related field preferred
Salary/Compensations
- $34.33/hr
