About the Role
Paradigm is seeking a Field Nurse Case Manager to cover Queens, Brooklyn, Bronx and surrounding areas to include Mt. Vernon, Manhattan, Staten Island, Yonkers, and North Bergen (NJ). This home/field-based position involves travel to various locations to manage the medical case management of work-related injuries.
Responsibilities
- Assessment, planning, coordination, implementation, and evaluation of injured/disabled individuals.
- Working with insurance carriers, medical care providers, attorneys, employers, and employees.
- Monitoring the progress of the injured worker.
- Reporting results back to the employer and insurance carriers.
Requirements
- Current, unencumbered Registered Nurse (RN) license in NY.
- Current, unencumbered Registered Nurse (RN) license in NJ preferred.
- Experience in case management of workers comp cases preferred.
- Recommended certifications (CCM, CRRN, CDMS, CRC) required within two years of the date of hire.
- Valid Driver’s License and good driving history.
