About the Role
The Social Worker will develop services using the Harm Reduction model of care, emphasizing tenant choice in service delivery. This role supports sobriety and engagement while providing trauma-sensitive case management, including assessment, treatment planning, and referrals.
Responsibilities
- Conduct complete bio-psycho-social assessments covering substance use, health, mental health, relationship abuse, social services, family, schooling, and work.
- Liaise with tenant's treatment providers with appropriate written consents to coordinate services.
- Complete individualized service plans with clear goals and outcomes to support tenants in maintaining housing.
- Provide case management services on daily living skills, crisis intervention, financial management, substance use, mental health, public benefits, and entitlements advocacy in consultation with tenants.
- Assist in establishing household and other services to support tenant's housing stability.
- Provide individual and group therapy to assist in overcoming dependencies, adjusting to life, or making changes.
- Perform all related and unrelated duties as assigned.
Requirements
- Must have experience in therapeutic intervention.
- Must have an understanding of utilizing community resources.
- Must have experience working with individuals who have been homeless and identified with a history of substance use.

- MSW, MHC, MPA, MPH, MA in Psychology or MS in Human Services required.
Salary/Compensations
- $60,000 - $64,000 / year dependent on experience
