About the Role
We are seeking an experienced Certified Rehabilitation Counselor to provide patient-centered care for individuals with complex neurological injuries. This role focuses on community resource coordination and functional reintegration, helping clients achieve maximal independence and vocational success within home and community settings.
Responsibilities
- Evaluate work and educational history, interests, and physical skills to determine vocational potential.
- Develop interdisciplinary vocational rehabilitation treatment plans based on evaluation goals.
- Administer direct vocational rehabilitation techniques and skill-building programs.
- Design pre-vocational programs to measure client skills and interests.
- Oversee individual treatment plans implemented by support staff.
- Design community reintegration activities to support vocational needs.
- Provide training to clients and relevant stakeholders, documenting all results.
- Perform thorough evaluations and set measurable objectives in collaboration with the rehabilitation team.
- Apply professional knowledge to facilitate problem resolution and goal achievement.
- Report progress to rehabilitation staff, clients, and family members.
- Observe and document changes in client condition to the clinical team.
- Maintain treatment documentation in accordance with the Plan of Treatment.
- Participate in team processes, case conferences, and family educational meetings.
- Guide Rehabilitation Specialists and Certified Nursing Assistants in plan fulfillment.
- Maintain strict confidentiality of client information.
- Collect data for program evaluation and improvement.
- Attend internal staff development and continuing education programs.
Requirements
- Master's degree in Rehabilitation Counseling or equivalent.
- Minimum of two years of vocational counseling experience with individuals with neurological impairments.
- Knowledge of complex neurologic injury and orthopedic rehabilitation techniques.
- Maintain professional license and/or certification as required by the state.
- Successful completion of pre-employment evaluation tests.
- Reliable transportation.
- Physical ability to perform job duties including lifting, standing, bending, stooping, stretching, walking, pushing, and pulling.
- Successful completion of a health examination prior to assignment.
