About the Role
Oversee and maintain the successful operation of the Individualized Residential Alternative/Intensive Care Facility serving Individuals with Developmental Disabilities. Expected to carry out responsibilities with professionalism, forward-thinking, and as a positive role model.
Responsibilities
- Demonstrate ability to problem solve, multi-task, and conduct research to resolve program concerns in a timely manner.
- Be available on-call 24/7 for emergent needs, triage, and support, with a flexible schedule and availability to deploy to the site as needed.
- Exhibit effectiveness in handling crisis situations, deploying resources, and communicating effectively to required parties.
- Provide training, direction, and supervision to Direct Support Professionals and the Assistant Manager.
- Maintain meticulous records to track attendance, compliance, and outcomes.
- Maintain own training compliance.
- Coordinate all services for Individuals, including linkages to day treatment, medical, psychiatric, and financial services, considering Individual choice.
- Ensure the program is prepared for External DQI Audits and conduct regular self-audits of records.
- Oversee and implement agency medication protocols, working collaboratively with staff and RN/LPN to ensure OPWDD Medication protocols are followed.
- Conduct mandatory staff meetings at least once per month with a prepared agenda and maintain minutes and sign-in sheets.
- Attend monthly Managers’ meetings, arriving timely and prepared to present program updates.
- Develop specific Residential Habilitation Plans/LRO/SRO for individuals, working with the Case Manager/QIDP to conduct reviews every six months.
- Maintain active target dates on the Residential Habilitation Plan/LRO/SRO necessary for service delivery.
- Coordinate the implementation of Residential Habilitation Goals LRO/SRO, including staff assignments, data collection, progress notes, and billing requirements.
- Work collaboratively with the clinical department (BIS) to complete/oversee ABC sheets and other behavioral tracking mechanisms, ensuring Behavior Plans are implemented.
- Maintain records and submit according to program due dates.
- Utilize Precision Care for all documentation and create monthly summaries based on progress notes.
- Identify areas of concern and appropriately address them with assigned team members.
- Manage staff schedules, required trainings, and the staff onboarding process.
- Participate in the interview process, documenting applicant tracking forms and forwarding to HR within 24 hours.
- Assist in actively recruiting for new team members as necessary.
- Approve timesheets via PayCom every Sunday for all assigned employees.
- Participate in the admission/discharge process, including trial visits and 30-day discharge follow-up reports.
- Confidently represent the agency to other service providers and community groups.
- Oversee cooking needs with healthy menu planning, food shopping/ordering, and meal preparation, incorporating dietary needs.
- Review the weekly menu, making any needed changes.
- Submit Weekly Food ledgers/receipts and Monthly Recreational receipts, van logs/receipts, etc.
- Maintain documentation of annual/bi-annual medical appointments.
- Accompany residents on medical appointments utilizing agency vehicle; assign to Residential staff where possible.
- Commit to professional development.
- Exercise sound judgment always.
- Keep up with a multitude of emails via Microsoft Outlook.
- Perform other duties via Microsoft Suites (Excel, Word, Teams).
- Effectively manage time, organize workspace, and demonstrate proactive planning/prioritization to accomplish responsibilities.
- Ensure organization of monthly recreation activities to promote independence, wellness, and community integration.
- Establish and maintain professional, interpersonal relationships.
- Develop constructive and cooperative working relationships.
- Efficiently handle complaints, settle disputes, resolve grievances, and offer mediation of staff and individuals.
Requirements
- A H.S Diploma (Bachelor’s degree Preferred) with at least three (3) years’ experience working with the Developmentally Disabled population.
- Graduate from an accredited college or university with a degree in the field of human services preferred.
