About the Role
The Care Manager II (CMII) is responsible for assessing clients with multiple medical and psychosocial needs, planning and monitoring services, and ensuring the provision of quality care to help aging adults and adults with disabilities maintain their quality of life.
Responsibilities
- Conduct comprehensive assessments and ongoing re-assessments of the client, including psychosocial, physical, mental health, environmental, and spiritual needs.
- Write comprehensive assessments.
- Develop and initiate the Community Living Plan with the client, ensuring it is client-centered, comprehensive, and consistent with program guidelines and policies.
- Conduct home visits, acute hospital, and skilled nursing facility visits, and escort clients to appointments as clinically indicated.
- Identify, arrange for, and monitor appropriate community services based on knowledge of Medicare, Medi-Cal, and other entitlement programs.
- Establish and maintain a care management relationship with clients and their support network, offering respect, dignity, and support.
- Provide crisis intervention, advocacy, problem-solving, and therapeutic interventions.
- Meet with clients at least monthly, or more often as clinically indicated, and review/modify their Community Living Plan on an ongoing basis.
- Document all case management activity via progress notes within 24-48 hours, adding new problems to the Community Living Plan as needed.
- Maintain required paperwork and follow a clear, concise, and consistent charting system for continuity of care.
- Evaluate client's Purchase of Service needs and follow up to ensure timely service provision.
- Educate clients and their support network about available resources.
- Establish and maintain open and effective communication with community providers, including physicians and other health/social service workers.
- Provide appropriate information on client care and program operations while maintaining confidentiality.
- Monitor the quantity and quality of services provided by other involved providers.
- Continuously evaluate clients' ability to remain safely at home and coordinate placement as appropriate.
- Terminate clients when appropriate, in collaboration with the client, caregiver, and involved services, and document the process.
- Participate in research studies and data collection as required.
- Participate in and promote Continuous Quality Improvement efforts.
- Attend and actively participate in team and program meetings, contributing to open communication.
- Utilize supervision appropriately, maintaining open communication and providing caseload updates.
- Incorporate the ethical and legal standards of the National Association of Social Workers into all interactions.
- Understand and apply the regulatory and procedural requirements of the Institute on Aging.
- Attend continuing education classes and/or in-service training to enhance knowledge and skills.
- Perform other reasonably related responsibilities as assigned.
Requirements
- One year of experience working with disabled adults and/or older adults.
- Experience with and understanding of the medical and psychosocial problems of functionally impaired adults and older adults.
- Experience working with individuals with mental and/or behavioral health diagnoses and substance abuse disorders is highly desired.
- Exceptional communication and presentation skills relating to functionally impaired adults and older adults, their support systems, and teams of health professionals.
- Demonstrates case management skills and experience in the community health care delivery system.
- Detail-oriented with good problem-solving skills and the ability to prioritize multiple tasks.
- Computer literacy required.

Experience Level

Education Level
- M.S.W. (Masters in Social Work) or another appropriate Masters level degree (MPH, MFT) with additional or specialized work experience (psychology, counseling, geriatrics).
- Alternatively, a BA or BS in Social Work or another appropriate major with a minimum of two (2) years of relevant social work experience and the ability to demonstrate autonomous work in conceptualizing and formulating biopsychosocial assessments, identifying care needs and necessary interventions, and then executing effective care interventions.
