About the Role

Niagara Hospice is seeking a compassionate RN to join our home care team as a Case Manager, providing comprehensive, direct patient care and plan of care oversight.

Responsibilities

  • Provide direct patient care and assessment, and oversight of plan-of-care development and implementation.
  • Identify and discriminate between physical and psychosocial signs and symptoms essential to effective execution and management of the nursing regimen.
  • Execute prescribed medical regimens and administer medications and treatments during home visits.
  • Evaluate signs, symptoms, and processes denoting individual interaction with actual or potential health problems.
  • Educate patients and families on palliative and end-of-life care and issues proactively.
  • Demonstrate respect for patient and family choices, values, and behaviors, and advocate for them.
  • Communicate patient progress to advanced practitioners, complete patient profiles/summaries promptly, and record patient condition, treatment, and response in the MISYS Homecare System.
  • Place referrals for necessary supportive services and act as a liaison to contracted hospitals and nursing facilities.
  • Demonstrate knowledge and competencies required for the position, including understanding of the NYS Nurse Practice Act and governing guidelines.

Requirements

  • RN Licensure in New York State
  • Graduate of an accredited school of professional nursing
  • Valid NYS Driver's License

  • Weekend required every 8 weeks
  • Involvement in holiday rotation

Experience Level

  • Minimum of three (3) years of nursing experience preferred

Salary/Compensations
