Responsibilities
- Complete Chronic Disease Management Visits (CDMVs), standalone or integrated within another VBC clinical pathway.
- Perform ESKD rounding, including limited and comprehensive visits, under the guidance of practice nephrologists.
- Manage ESKD holistic patient care, including patient activation, depression, high-risk hospitalization, transition of care, and co-morbid medical issue oversight.
- Provide ESKD patient education.
- Conduct ESKD comprehensive disease management annual visits.
- Collaborate with nephrologists, dialysis center IDT, Kidneylink IDT, hospital discharge care coordination teams, PCPs, and sub-specialists as clinically indicated.
- Provide CKD clinical care and kidney disease education.
- Perform physical examinations to evaluate health and diagnose illnesses.
- Order and assess diagnostic tests, such as x-rays and laboratory tests.
- Determine, plan, and initiate appropriate treatment, adjusting as necessary.
- Prescribe medications.
- Document patient medical information and maintain electronic medical records.
- Educate, instruct, and counsel patients and families on health problems, treatment plans, and home care, promoting preventative health and positive behaviors.
- Request consultation or referral with other health care providers and collaborate with Kidneylink RN Care Manager.
- Actively promote GUEST customer service standards and develop effective relationships.
- Participate in team efforts and perform duties in accordance with company policies.
- Maintain regular and reliable attendance.
Requirements
- Current license to practice as an Advanced Practitioner in the state performing duties.
- Firm understanding of physician practice protocols, policies, and operations.
- Necessary specialty certification.
- 2 years of clinical work experience.
- Excellent written and verbal communication skills.
