About the Role

LindenGrove New Berlin, an Illuminus Community, provides assisted living, memory support, short-term rehabilitation, and long-term skilled care. We are dedicated to offering holistic, transformative care that helps residents rediscover their light.

Responsibilities

  • Promote effective communication with residents, patients, clients, and their families.
  • Maintain a positive community image aligned with company mission, vision, and values.
  • Perform care duties as directed by the Director of Nursing A/L.
  • Notify the on-duty nurse promptly regarding any changes in resident status.
  • Provide services consistent with the established resident service plan.
  • Participate in required training and complete 15 hours of in-service training annually.
  • Utilize proper body mechanics for all tasks.
  • Perform only tasks for which training has been provided.
  • Practice standard precautions at all times.
  • Document services accurately and in a timely manner.
  • Cooperate with scheduling to ensure adequate coverage.

  • Ability to relate well to residents, patients, clients, and their families.
  • Ability to maintain and protect the confidentiality of information.
  • Ability to exercise independent judgment and make sound decisions.

  • Reliable attendance.
  • Completed documented proof of CBRF Training as outlined in HFS-83.
  • Must be at least 18 years of age.

  • $18.25 - $19.50 per hour
  • $2.00 shift differential for PM and night shifts
