About the Role

The EIS Case Manager will improve access to and retention in medical and support services for people living with HIV. This role supports the JPAFHC clinic system by providing client-centered services, HIV testing, counseling, care coordination, and linkage to social support services, aiming for timely and coordinated access to care.

Responsibilities

  • Conduct HIV Testing (internally and in the community), including Pre-test and Post-test counseling.
  • Conduct initial pre-screening of clients to determine eligibility for case management services.
  • Facilitate linkage to appropriate services for patients based on presenting problems.
  • Assist patients with completing required documentation.
  • Conduct risk reduction counseling, partner counseling, and provide referral services.
  • Complete client initial assessments and reassessments to develop a comprehensive care plan with short and long-term goals.
  • Re-evaluate care plans at least every 4-6 months.
  • Refer and link patients to services that promote positive health outcomes and treatment adherence.
  • Monitor patient linkage to services.
  • Support medical treatment plan implementation through health education and treatment adherence support.
  • Address treatment adherence barriers.
  • Participate in Viral Load Case Conferencing to support care coordination and address barriers.
  • Evaluate service effectiveness based on client outcomes.

Requirements

  • Bachelor’s Degree in Human Services, Social Work or related field preferred, but equivalent community work experience and/or other certifications will be considered for exceptional candidates.
  • At least 2 years of case management experience preferred.
  • Experience in HIV, healthcare, and/or social service settings preferred.
  • Computer literacy with proficiency in MS Word, Excel, and PowerPoint.
  • Experience with Electronic Medical Records preferred.
  • Demonstrated organizational, interpersonal, oral, and written communication skills.
  • Ability to handle multiple assignments simultaneously.
  • Comfort working independently and collaboratively in a clinical environment.
  • Cultural competency and comfort working with LGBTQ and PLWH populations.
  • Bi-lingual (English/Spanish) preferred.
