About the Role

Provide 1:1 adult neurorehabilitation in patients’ homes and community settings. Serve as the primary treating SLP from evaluation through discharge. Design a flexible work schedule that fits your lifestyle, seeking approximately 15 hours per week across 3 days.

Responsibilities

  • Perform evaluations and develop treatment plans for patients with speech, language, cognitive, and swallowing disorders.
  • Re-assess treatment plan effectiveness, attend staffing meetings, and participate in family conferences.
  • Conduct discharge evaluations, recommend home and community safety measures, and provide referral and educational resources.
  • Communicate patient needs and progress to the treatment team, physician, patient, and family members.
  • Collaborate with other disciplines to ensure coordinated care and enhance patient outcomes.

Requirements

  • Minimum of a Master’s Degree from an accredited Speech Language Pathology program.
  • Valid Speech Language Pathology license in the state(s) of practice.
  • Current CPR Certification in accordance with state regulations.
  • A minimum of one year’s work experience as a Speech Language Pathologist.
  • Demonstrates knowledge of rehabilitation techniques related to complex neurological injury preferred.
  • Effective and professional verbal and written communication skills.
  • Ability to lift 50 pounds.
  • Ability to move, lift, or transfer patients, potentially up to 100 pounds, following safety procedures.
  • Duties require fine motor skills, visual acuity, and walking/standing for extended periods.
  • Additional physical requirements include frequent pushing/pulling, bending/stooping, reaching, kneeling, and positioning.
  • A health screen or examination may be required to verify physical capability for assigned duties with or without reasonable accommodations.
