About the Role

The Case Manager is a key member of the team, providing care coordination, health education, and health promotion services. This role addresses barriers to care and offers supportive services to help patients manage their illnesses and access necessary medical, psychiatric, and social services, in alignment with Ryan Health's mission and goals.

Responsibilities

  • Ensure coordination and follow-up of medical, psychiatric, and social services.
  • Provide reminders for appointments and follow-up contacts for missed appointments.
  • Conduct home, hospital, or community site visits and escort activities, particularly for hard-to-reach patients.
  • Provide crisis intervention, assist in developing safety plans, and make referrals for emergency services when necessary.
  • Educate patients on early medical intervention, preventive health care, stress reduction, and adherence to treatment.
  • Provide prevention counseling and education services for various patient groups and their significant others.
  • Document all patient encounters and services in the electronic medical record.
  • Attend required meetings, supervision, case conferences, and in-service seminars.
  • Immediately report critical or unusual problems to the Program Manager/Senior Director of Support Services.
  • Conduct educational activities to promote program services and provide presentations to staff.
  • Practice accepted infection control measures.
  • Perform other duties as assigned.

Requirements

  • BSW or BA in Health or Human Services, Health Education, Mental Health, Counseling, or related field.
  • Experience working with people in a health care or social service setting.
  • Computer skills.
  • Effective communication and writing skills.
  • Bilingual (English & Spanish) preferred.
  • Master’s Degree in a related field preferred.
  • Professional, courteous, and respectful attitude in dealing with patients.
  • Strong ability to develop relationships with patients from diverse backgrounds.
  • Demonstrate knowledge and skills necessary to provide age-appropriate care.
  • Ability to take responsibility for their own learning.
  • Commitment to exceeding patient expectations.
  • Works well with others and contributes input to improve outcomes.
  • Provides coverage for other case managers under the direction of the Supervisor of Case Management.
  • Asks for and provides feedback in a tactful and respectful way.
  • 90-day probation period.
