About the Role

As an NP/PA PM&R Clinician with Comprehensive Rehab Consultants, you will work closely with the post-acute therapy team to optimize rehabilitative outcomes and promote patient independence within skilled nursing facilities.

Responsibilities

  • Provide specialized Physical Medicine and Rehabilitation services as a rehab consultant
  • Represent the company through clinical leadership and collaboration with interdisciplinary care teams
  • Collaborate with PCP teams to manage conditions such as CVA, progressive neurological conditions, and postoperative orthopedic surgeries
  • Work alongside PT, OT, and SLP teams to improve patient function and quality of life
  • Track patient progress in therapy
  • Assess and manage barriers to therapy including pain, spasticity, and neurogenic conditions
  • Attend multidisciplinary meetings and bridge the gap between therapy and internal medicine
  • Engage in culture-building activities and ongoing education

Requirements

  • Nurse Practitioner, APRN, or PA license
  • Connecticut state license
  • Ability to work in the U.S.
  • Reliable commute to New London, CT or Plainfield, CT
  • Passion for post-acute healthcare and the geriatric population
  • Confidence to function autonomously
  • Desire for continuous improvement and ability to embrace change
  • Strong interpersonal skills
