About the Role
As an NP/PA PM&R Clinician with Comprehensive Rehab Consultants, you will work closely with the post-acute therapy team to optimize rehabilitative outcomes and promote patient independence within skilled nursing facilities.
Responsibilities
- Provide specialized Physical Medicine and Rehabilitation services as a rehab consultant
- Represent the company through clinical leadership and collaboration with interdisciplinary care teams
- Collaborate with PCP teams to manage conditions such as CVA, progressive neurological conditions, and postoperative orthopedic surgeries
- Work alongside PT, OT, and SLP teams to improve patient function and quality of life
- Track patient progress in therapy
- Assess and manage barriers to therapy including pain, spasticity, and neurogenic conditions
- Attend multidisciplinary meetings and bridge the gap between therapy and internal medicine
- Engage in culture-building activities and ongoing education
Requirements
- Nurse Practitioner, APRN, or PA license
- Connecticut state license
- Ability to work in the U.S.
- Reliable commute to New London, CT or Plainfield, CT
- Passion for post-acute healthcare and the geriatric population
- Confidence to function autonomously
