About the Role
CBI Home Health is seeking a Social Worker to join their team in the North York region. This role contributes to providing specialized community services and integrated care, helping individuals live safely and independently.
Responsibilities
- Deliver the right care at the right time.
- Travel within the Hawkesbury community as part of the CBI Health Home Health Services team.
Requirements
- Bachelor of Social Work or a Master of Social Work.
- Registered member in good standing with the Ontario College of Social Workers and Social Service Workers.
- 1 to 2 years of community experience is an asset.
- Passionate, empathetic, use good judgement and apply critical thinking when making client decisions.
- Excellent communication skills and ability to work in collaboration within an interdisciplinary team as well as work independently.
