About the Role

CBI Home Health is seeking a Social Worker to join their team in the North York region. This role contributes to providing specialized community services and integrated care, helping individuals live safely and independently.

Responsibilities

  • Deliver the right care at the right time.
  • Travel within the Hawkesbury community as part of the CBI Health Home Health Services team.

Requirements

  • Bachelor of Social Work or a Master of Social Work.
  • Registered member in good standing with the Ontario College of Social Workers and Social Service Workers.
  • 1 to 2 years of community experience is an asset.
  • Passionate, empathetic, use good judgement and apply critical thinking when making client decisions.
  • Excellent communication skills and ability to work in collaboration within an interdisciplinary team as well as work independently.
