About the Role
As a Dietitian in our Integrated Care Program, you’ll play a key role in recovery by providing nutrition support during the transition from hospital to home. Collaborate with an interdisciplinary team, prevent readmissions, and empower patients to thrive in their community.

Requirements
- Registered as a member in good standing with the College of Dietitians of Ontario.
- Familiarity with tube feeds is considered an asset.
- Focus on embracing person and family-centered care.
- Passionate, empathetic, use good judgement and apply critical thinking when making client decisions.
- Excellent communication skills and ability to work in collaboration within an interdisciplinary team as well as work independently.
- Value the role a dietitian plays in keeping clients at home.
- Ability to travel within the community as part of the CBI Health Home Health Services team.
- Bilingual language skills in English and French are an asset.
