About the Role

The Clinical Transition Specialist is an experienced sales professional responsible for educating patients, families, and facility staff about Option Care's services and products, ensuring proper patient placement in home health care settings. They assess patients, gather preadmission information, collaborate with internal and external partners, and build relationships with referral sources to grow referral rates and achieve sales goals.

Responsibilities

  • Proactively maintain and grow relationships with referral sources to increase sales and patient starts.
  • Serve as a key point of contact and representative of Option Care, providing education, assistance, and service to referral sources.
  • Interact with area service providers daily to sell Option Care services for discharging patients.
  • Conduct patient assessments to ensure viability for alternative site care.
  • Use sales techniques to educate patients, families, and facility staff about Option Care services and products.
  • Provide hands-on, in-person education to patients and caregivers for timely discharge and therapy independence.
  • Communicate effectively with agency staff, medical teams, patients, and families throughout the discharge process.
  • Collect, review, and complete pre-admission information and secure related signoffs.
  • Partner with Account Manager and Regional Sales Director to create and execute area business and growth plans.
  • Communicate frequently with Account Manager to discuss opportunities, assess progress, and provide feedback.
  • Partner with Account Manager and marketing staff to deliver educational and promotional programs.
  • Review patient medical records for pertinent medical history and insurance information.
  • Proactively initiate care transition coordination with referral sources and internal partners for seamless patient transitions.
  • Participate in data collection for therapy start and patient tracking processes.
  • Maintain confidentiality of patient and proprietary information, observing legal guidelines.
  • Provide oversight and input to providers regarding patient assessment, treatment process, and transition to home care.
  • Serve as a point of contact, coordination, and communication with other providers.
  • Make arrangements for any special medical supplies or appliances to be available.

Requirements

  • Licensed Registered Nurse (RN) or licensed Pharmacist in the state of practice.
  • At least 2 years of experience in the healthcare industry.
  • Experience establishing and maintaining relationships with individuals at all levels of the organization, in the business community, and with vendors.
  • Experience applying knowledge of standard practices for all services offered and current relevant standards (e.g., ACHC, URAC).
  • Experience providing customer service to internal and external customers, meeting quality standards, and evaluating customer satisfaction.
  • Basic PC skills: proficient in internet, email, Microsoft Word, Microsoft Excel, Microsoft PowerPoint.
  • Experience in identifying operational issues and recommending/implementing strategies for resolution and improvement.
  • Access to a reliable means of transportation for travel to care facilities, home visits, and multiple hospitals.
  • If using a personal vehicle, a valid driver’s license and proof of insurance are required.
