About the Role
The Case Coordinator is responsible for developing individual support plans for clients in the Special Services at Home Program and Passport Program. This role involves assessing client and family needs, matching support workers, and providing ongoing support to clients in the community.
Responsibilities
- Interpret and complete standardized assessment tools (e.g., interRAI CHA, Supports Intensity Scale).
- Conduct client assessments using conversation-based methodology.
- Develop comprehensive therapeutic rapport.
- Identify and prioritize environmental risks to client health and safety.
- Develop comprehensive care plans.
- Facilitate access to client-identified services and navigate to appropriate resources.
- Make referrals to community and health resources.
- Consult with partners in care service plans with client consent.
- Monitor client progress and adjust care plans.
- Review client care plan expenditures and support clients in managing budgets.
- Collect and report relevant data on program outcomes.
- Adhere to privacy policies and standards.
- Represent potential providers, stakeholders, and agencies to clients.
- Mediate and problem-solve with multiple parties.
- Screen and match support workers, facilitate interviews and contracts.
- Meet with family and support worker to develop service goals and negotiate terms.
- Provide referrals and support in emergency and urgent situations.
- Promote the goals and values of VON.

- Work in collaboration with other staff in a team approach.
Requirements
- Minimum of 3 years related experience in a home and community care organization or social services program.
- Experience in case management.
- Experience in building external and community relationships.
- Demonstrated experience in completing validated common assessment tools and maintaining competency.
- Broad working knowledge of community resources.
- Knowledge and skills in comprehensive assessment and the ability to maintain proficiency.
- Must have access to a personal vehicle for business use.
- Must possess a current driver’s license.
- Must possess proof of vehicle insurance.
- A current and original copy of a satisfactory Criminal Records Check.
- A Vulnerable Sector Search and/or a Child Abuse Registry Check may be required.
- The use of Personal Protective Equipment (PPE) may be required.
- Requires a detail-oriented approach in a dynamic environment.
- Requires physical activity including lifting, carrying, bending, reaching, kneeling.
- Individuals are required to walk, sit, stand, and climb stairs.
- Some tasks requiring fine hand movements.
