About the Role

Extended Home Care of NY seeks an experienced, reliable home care Physical Therapist to conduct home visits in the Bronx and Suffolk for the IDD population. This role involves providing physical therapy services to patients based on a physician's plan of care, including assessing therapeutic/rehabilitative/functional status and contributing to the total plan of care.

Responsibilities

  • Evaluate home environment.
  • Assess muscle strength, mobility, gait, and range of motion for rehabilitation potential.
  • Direct physical therapy treatment.
  • Instruct patients and families/caregivers on therapeutic appliance use and care.
  • Determine priority needs for physical therapy.
  • Report patient's reaction to treatment or changes in condition to the physician.
  • Initiate physical therapy programs and instruct personnel/family/caregivers on therapy phases and goals through case conferences.
  • Train patients in prosthetic device use.
  • Identify and refer patient and family/caregiver needs for other home health services.
  • Prepare and submit clinical and progress summaries based on goal attainment.
  • Participate in patient discharge planning.
  • Provide physical therapy consultation to home families/caregivers as indicated.
  • Provide in-service education programs for nursing organization personnel as needed.
  • Participate in peer consultation process.
  • Supervise Physical Therapy Assistants according to organization policy and state regulations.
  • Instruct, supervise, and evaluate home health aide care every two weeks when therapy is the sole skilled service.

Requirements

  • Degree from a baccalaureate or master's program in physical therapy approved by an accredited organization.
  • Licensed to practice as a physical therapist within the State, if applicable.
  • Two years of appropriate experience as a physical therapist.
  • Community/home health experience preferred.
  • Demonstrated good verbal and written communication skills.
  • Demonstrated good organization skills.
  • Current CPR certification.
  • Licensed driver with an insured, good working automobile per state/organization requirements.
