About the Role
United Hospice, Inc. is a community-based, not-for-profit hospice providing care and support to patients and their families. We aim for a community where individuals and their loved ones facing serious illness maintain dignity and hope while receiving optimal hospice care. We are seeking NYS Certified HHAs for our Joe Raso Hospice Residence on a per diem basis for the overnight shift (11:00 pm to 7:00 am).
Responsibilities
- Follow the Plan of Care established by the hospice nurse
- Actively participate in coordinating all aspects of patient care, adhering to current professional standards and practice
- Perform health care procedures monitored by the hospice nurse (e.g., reinforce clean dressings, use bed pan, check urinary catheter)
- Provide physical care to patients as outlined in the HHA care plan (e.g., bathing, toileting, skin care, ADLs)
- Assist patients with ambulation, transfers, turning, and positioning
- Maintain a safe patient environment and ensure proper equipment use (e.g., wheelchair, Hoyer lift) under guidance of primary nurse, PT, OT
- Assist patients with prescribed exercises
- Plan and prepare meals for patients; feed patients when necessary
- Perform light housekeeping tasks essential to patient healthcare; wash patient clothes as needed
- Attend team meetings as needed
- Attend 12 hours of documented annual training
- Incorporate commitment to customer service and continuous quality improvement
- Promote positive co-worker and teamwork relationships

- Accept additional responsibilities as assigned
Requirements
- NYS Home Health Aide certification; maintain certification with 12 hours of annual approved continuing training
- Working knowledge of basic first aid
- Basic understanding and commitment to the hospice philosophy of care
- Ability to work as a team member and use supervision effectively
- Ability to maintain a high level of confidentiality concerning patient and family information
- Ability to relate to a diverse population and workforce in a personable and effective manner
- Ability to understand verbal and written directions
- Ability to communicate effectively verbally with patients, families and staff; able to write and document as needed
- Proficiency with computer, email, EMR systems, and MS Office; capable of data entry
- Access to a vehicle for daily patient-to-patient travel
- Physical agility for repetitive tasks including walking, running, bending, stooping, climbing stairs, kneeling, crouching, reaching, sitting, and twisting
- Repeatedly lift over 50 lbs throughout the workday
